MedGate-Fusion Combines Clinical Narratives and Biomarkers for Five-Year Stroke Risk Stratification

Summary

MedGate-Fusion is a multimodal gated architecture designed to improve prospective stroke risk stratification in primary care, where early signals are spread across routine biomarkers and unstructured clinical narratives. The system combines Transformer-based embeddings of first-encounter narratives with ten routinely recorded risk markers. Using electronic medical records from the Canadian Primary Care Sentinel Surveillance Network, the researchers started with 808,921 encounter-level observations and retained 102,736 unique patient records with usable narratives and sufficient data for a five-year stroke outcome. To reduce explicit target leakage, stroke-related terms were redacted from the notes before semantic encoding.
