AI-Assisted Medical Coding Linked to Nearly $1 Billion in Questionable Hospital Charges
Summary
Artificial intelligence is increasingly being used by hospitals and insurers to code diagnoses, review claims and manage medical billing. The Blue Cross Blue Shield Association estimated that hospitals’ use of AI-assisted coding contributed to $942 million in additional costs for its health plans from 2023 through 2025. The insurer said many of the extra charges came from secondary diagnoses that moved patients into higher-paying reimbursement categories, although it stopped short of attributing the entire increase to AI. About 60% of hospital systems began using AI coding tools during the period, and roughly 70%, or $653 million, of the billing identified by the insurer involved added diagnoses that were not accompanied by a change in care. Experts said some diagnoses may be legitimate and simply better documented, while others may not affect treatment but can increase payment. BCBSA warned that more complex coding could eventually raise premiums and out-of-pocket costs. The American Hospital Association disputed the analysis, saying patients are older and more medically complex and that AI can help providers capture conditions appropriately; it also criticized insurers’ automated downcoding and denial practices. BCBSA said its own AI-assisted claims reviews include human clinical review for clinical denials. Analysts described the opposing use of AI by hospitals and insurers as an expensive administrative “arms race” that could add burden without improving care. At the same time, healthcare advisers said automation can reduce physicians’ paperwork, improve work-life balance, increase documentation accuracy and help revenue-cycle teams handle appeals. They cautioned that better documentation is difficult to separate from misuse, user error and overcoding. Experts called for human oversight, audits of AI-generated codes and a focus on whether the technology improves access or quality rather than merely shifting financial advantage between organizations.
